Services as required at the Warm Springs Health and Wellness Center
(Warm Springs Service Unit) to provide Optometry support services
for eligible patients of the Warm Springs Indian Health Center. To
provide Optometry support services up to five (5) days per week at
Warm Springs Health and Wellness Center as needed. Within specialty
area, assumes responsibility for the diagnosis, prevention, and
therapy, in the capacity of an optometrist.Optometry support to be
provided shall include, but is not limited to the following:1.
Diagnosis of optometric and ocular problems through external and
internal physical evaluation of the eye and adnexa; detects
neuromuscular impairment of binocular function; treats visual
disorders through the prescription of corrective lenses such as
glasses, contact lenses, or low vision aids, vision therapy, and
pharmacological means; treats unusual visual disorders through the
application of advanced optometric techniques; may consult with
Area or I.H.S. Optometry Specialists on complex cases; refers
patients requiring medical services or surgery to the appropriate
medical specialty, refers patients requiring other health services
to appropriate health professionals; provides emergency eye
services including diagnosis and treatment of ophthalmic trauma or
pathology, on an outpatient basis; provides vision and eye care
education and counseling to patients. Responsible for recording all
patient care examinations and procedures in appropriate health
records in compliance with professional community standards and
other available guidelines.2. Provides Head Start, school vision,
and diabetic retinopathy screenings; maintains liaison with and
provides advisory eye services to other professional personnel on
patients; provides vision safety services for the Service Unit;
provides optometric and eye specialty clinics; acts as community
educator providing vision education and in-service training to
other health professionals and the public; develops and executes
community clinics providing complete optometric care.3. Any
required treatments shall be provided or referred appropriately
following evaluation of the patient's ancillary test results.4. The
eye care provider shall work with the optometry clinic support
staff to arrange the scheduling.5. The optometrist shall prescribe
drugs and tests necessary to provide including the use of radiology
and laboratory i. The optometrist shall become familiar with and be
guided by the Indian Health Service formulary and shall follow IHS
procedures when prescribing drugs ii. The optometrist authorized to
prescribe pharmaceuticals shall do so according to the availability
of drugs listed in the IHS formulary iii. IHS pharmacy services
will provide instructions to the prescribing contract optometrist
as to the substitution of generic drugs for prescribed drugs iv.
The service unit pharmacy must approve drugs, not on the
formulary6. Contractor to refer patients to appropriate contracted
medical or other government facilities, providing full clinical
information for care and diagnostic procedures that cannot be
adequately provided at the local Indian Health Service facilities.
Coordinates and integrates information obtained into the health
record at the Service Unit, and assures the provision of timely
follow-up care as required, as pertaining to optometry. All
referrals shall be done utilizing the Patient Referred Care (PRC)
process. The PRC Committee utilizing PRC guidelines will review
these referrals.7. The Contractor shall assure the preparation of
appropriate and timely medical records for all patients seen to
assure the accumulation and organization of all pertinent clinical
data needed to provide comprehensive optometry care. Coordinate
clinical reports and correspondence concerning patients under
his/her care to physicians, hospitals, and other individuals and
facilities.8. The Contractor shall manage all aspects of optometry
care electronically, using knowledge and skill of existing
automation equipment required to support all clinical/ medical
programs i.e. IHS-EHR, Microsoft Office, Outlook, etc.9. The
Contractor will work closely with allied health professionals (i.e.
community health nurses, social workers, health educators, etc.) to
utilize and coordinate the services of other health care
professionals in the management, as needed.10. Strive for
excellence in providing general optometry medical services.11.
Comply with all IHS facility infection control and safety
procedures, practices, and standards12. Respect and maintain the
basic right of patients, to include but not limited to personal
dignity
Requirements:
The provider contractor shall maintain the following qualifications
as a minimum according to their individual training and
certification:i. Optometry Graduate Training: Must possess a degree
of Doctor of Optometry, approved by accrediting bodies recognized
within the United States or Canada.ii. Board Certification: current
board certification in Optometry.iii. Licensure: Candidates must
have a permanent, full, and unrestricted Licensed to practice
optometry in a State, the District of Columbia, the Commonwealth of
Puerto Rico, or a territory of the United States. State licensure
must permit independent practice.iv. The provider is recommended to
hold DEA, but not required.v. Experience: Board Certification in
Optometryvi. The Contractor shall complete, sign and return the
required forms: Certification of Immunization Declaration of
Employment Certification of Education and Insurance Addendum to
Declaration for Federal Employment (OF 306) Indian Health Service
