High Lakes Health Care, part of Praxis Health, is looking for a professional Population Health and Triage Registered Nurse to join our team!


The Population Health and Triage Registered Nurse will participate in the development and support of a cross-regional patient phone triage system within Praxis Health and lead population health initiatives within the organization.  The coordinator will be asked to oversee staffing, operations and provide ongoing knowledge to refine the triage system.  The coordinator will be responsible for phone triage and other related duties as demanded by the department.  The coordinator will collaborate with leadership and providers to expand the department scope of practice, ensure quality of operations and provide expert insight into system needs for optimal functionality and compliance with local, state and federal regulations and standards.  The coordinator will serve as the point person for population health initiatives within the organization.

  • Monitors and responds to triage calls received on the nurse phone line and through patient portal messages.
  • Using established protocols:
    • Talks directly to patients on the telephone and then directs them to the appropriate place for care or offers home care advice.
    • Determines urgency of seeing the patient based on protocol questions and incorporating data gathered from the patient’s chart.
    • Sends patients with high-risk chief complaints such as chest pain, abdominal pain, or severe headaches to ER or immediately or arranges for ambulance.
  • Ensures accurate notes of all consultations and treatments are recorded in the patients’ record via the organization’s Ambulatory Electronic Health Record system.
  • Coordinates arranging appointments for patients who do not need to go to ER but need to see a clinician.
  • Consults with clinicians as needed to help determine best disposition for patient if unclear based on protocols.
  • Helps develop messaging for common information requests from patients, e.g., “Do I need a flu shot every year? Do I qualify for a COVID-19 vaccination? When are you giving these shots?”
  • Serves as a clinical resource and professional role model for other clinical and front desk staff.
  • Prioritizes work tasks and services and completes tasks in a timely manner.
  • Assists in medical chart reviews and compiling data for audits and reports.
  • Assists leadership with developing policies and procedures related to position.
  • Communicates appropriately and tactfully with staff, consultants, patients, significant others, and community.
  • Reduce conflict and increase patient satisfaction whenever possible through timely response to calls.
  • Participates in staff, planning, in-service, and other meetings as needed.
  • Administers medications/vaccinations and performs patient care services as ordered within the scope of practice.
  • Performs other duties as assigned.

POPULATION HEALTH FUNCTIONS: The Population Health Coordinator performs the following essential functions within the framework of Praxis Health’s organizational structure.

  • Participates and leads Praxis Health population health initiatives, which may include gathering data to identify unmet patient and public population health needs, prepare reports on findings, and provide interpretations/recommendations for developing a Praxis Health Strategic Plan.
  • Oversees in development, implementation and evaluation of programs and activities for a variety of populations (patient and public) to promote health and prevent disease.
  • Lead administrative efforts to address patient and public population health needs including strategic planning, assisting with grant writing/grant management, and maintaining communication with internal and external stakeholders.
  • Work closely with Praxis Health leadership, Deschutes County and other population health leaders to organize response efforts to prevent, minimize, and contain adverse health events and conditions, including communicable diseases, outbreaks, chronic diseases; environmental hazards and other health disparities.
  • Collaborate with medical staff to establish Praxis Health patient population appropriate care initiatives.
  • Assures that the delivery of population health services are evidence-based and/or best practices, and build the scientific basis of population health.
  • Collaborates in the development of evidence-based population health practices and programs in collaboration with universities, other health agencies, businesses, trade associations, staff and the public.
  • Organize and facilitate meetings, training and activities to promote population health initiatives.
  • Regularly request feedback from project participants and peers to improve programming and processes.
  • Provides leadership and expertise focused on Praxis Health needs assessment surrounding population health.
  • Evaluates the effectiveness of adopted programs and strategies surrounding population health and community health needs.
  • Performs other duties as assigned.



  • Collaborates with leadership on development, implementation and training related to clinic goals, policies, procedures, mission, and work standards.
  • Supervision of clinic staff.
  • Maintain performance standards of supervised employees including performance evaluations, discipline, training and development. Ensure adequate staffing levels through timely and effective recruitment.
  • Lead by example through an expertise in daily operations.
  • Delegation and monitoring of all office responsibilities.
  • Effective and timely staffing, training, and evaluating employees.
  • Guides employee actions by researching, developing, writing, and updating office policies, procedures, methods, and guidelines.
  • Understand and adhere to HIPAA policies, procedures and regulations to assure patient confidentiality.
  • Updates job knowledge by participating in educational opportunities; reading professional publications and appropriate e-mail updates.
  • Work with other supervisors and manager to facilitate cross-training and ensure inter-department communication and cohesion.
  • Other duties may be assigned.


Candidate must comply with all state, local and federal rules, guidelines and requirements with respect to safety and health.   • Minimum two years of experience in direct patient care, preferably in an outpatient care setting;
  • Minimum of two years increasingly responsible management, experience preferably in an outpatient setting.
  • Experience in an urgent care center preferred
  • Excellent verbal and written communication skills, including strong organizational, detail and interpersonal skills.
