Typical pay range: $21.16 - $29.22
This position is eligible for a combined sign on and retention bonus in the amount of $3,000. based on experience. To qualify for the bonus, candidates must start prior to 10/30/2021 AND must not have been employed by St. Charles Health System in the last 6 months.

DEPARTMENTAL SUMMARY: St. Charles Population Health department serves as the core design and facilitation team for health system population health and management services. The Population Health department is integrally linked to payer contracting, care coordination in the inpatient and outpatient setting, local and regional health plans, community needs and integration, public health and regional non-profit entities, and data analytics.

POSITION OVERVIEW: The Patient Advocate & Medication Support Specialist - Pharmacy Technician is responsible for gathering information from the patient, medical team, medical records and other sources as required to successfully complete patient assistance applications to reduce barriers and improve medication compliance. The Pharmacy Technician will also be solely responsible for ensuring patient assistance medication refill requests are completed in a timely manner to reduce gaps in care. In addition to this primary role, the Pharmacy Technician will be trained on St. Charles medication refill protocol and will assist the Pharmacy Technician refill team with refilling chronic medications as required. Additional duties may include review of medication gap lists as an extension of the Ambulatory Care PharmD team. This Pharmacy Technician will also support ambulatory care pharmacy operations. This position does not directly manage any other caregivers.

ESSENTIAL FUNCTIONS AND DUTIES:

Responsible for gathering all required information for submission of patient assistance applications across St. Charles Family Care clinics.

Aids providers, care team, and/or ambulatory care pharmacists in identifying patient assistance programs or alternative cost saving methods of obtaining medications for improved patient medication adherence.

Requests information from insurance companies and pharmacies on behalf of prescribers regarding possible alternative therapies and provides patients with timely status communications. 

Assists ambulatory care pharmacists with gap lists by contacting patients to determine compliance to prescribed medications, coordinates with PharmD if non-compliance is identified.

Supports clinics and health care providers by reviewing patient charts to determine appropriateness of medication refills for chronic conditions as outlined by the St. Charles Family Care Pharmacy Technician Refill Protocol.

Utilizing St. Charles Family Care Refill Protocol, creates and routes electronic health record (EHR) encounters from information received from pharmacy faxes, pharmacy electronic refill requests, patient phone calls, and EHR patient portal messages, to send in medications to pharmacy or when appropriate to providers or ambulatory care pharmacists. 

Conveys professional communication with providers at all times both orally and written. Responsible for proper communication and documentation of each medication refill within EHR utilizing appropriate templates within Epic.

Assists prior authorization (PA) department by receiving pharmacy PA requests and assesses if alternative medications with similar/appropriate efficacy are available to circumvent PA process.

EDUCATION

            

Required: High School diploma or GED

LICENSURE/CERTIFICATION/REGISTRATION

Required: Active CPhT license. Valid Oregon driver's license and ability to meet SCHS driving requirements. Ability to travel to business functions/trainings/meetings and all SCHS worksites.

EXPERIENCE

  • : One (1) year paid customer service experience. Must be able to learn how to appropriately and accurately document clinical information in EPIC during introductory period.
  • : Two (2) years related experience in billing, insurance benefits, and prescription refill coordination. Experience using EPIC, an electronic health record. Experience working in a Joint Commission Accredited health care facility, and/or an ambulatory care facility designated as a Patient-Centered Medical Home.

PERSONAL PROTECTIVE EQUIPMENT

Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
