TITLE: Physician - Immediate Care

DEPARTMENTAL SUMMARY: The Immediate Care Centers provide urgent care services to all presenting patients with Emergency Severity Index (ESI) of 4 and 5 with some ESI 3's as may be deemed appropriate. Patients who present requiring a higher level of care will be assisted in arriving at the appropriate higher level of service. The ICCs interact collaboratively with primary care, emergency care and specialty providers from within St. Charles as well as other non- associated entities.

POSITION OVERVIEW: The St. Charles Immediate Care Centers are staffed by physicians and Advanced Practice Providers (APPs) who are skilled in providing urgent and acute care. Requirements include the ability to work in a collaborative environment with advanced practice providers to provide rapid and efficient care for minor emergencies and illnesses. Responsible for with the clinical supervision of APPs and signing PA charts is essential. Other duties may include performing basic physical exams such as sports physicals and occupational exams. The ability to work efficiently and refer appropriately is crucial. The ability to stabilize patients who present needing a higher level of service while awaiting transport is required. The ICC's function as a unified department and shifts will be rotated among the locations. Lab and X-ray are integrated into the department. Physicians should have emergency department, urgent care, or primary care with substantial acute care experience.

  • Work schedule: Physician to follow a shift-based model with a base reconciliation of .75 FTE scheduled at 114 shifts per calendar year. Shifts are 12 hours in length. Shift counts will be adjusted accordingly by the providers FTE.
  • Call expectation: Physicians are not expected to have call expectations in the Immediate Care however it is the expectation that 95% of all shifts will be covered due to call outs, unexpected absences, or any unfilled shift to ensure that clinics have proper staffing at all times.
  • Physician Practice Location: Physician will render the Professional Services at such SCHS Facilities as may be directed by SCMG, and at such other facilities and locations as may be established by SCHS or SCMG from time to time. It is anticipated that the primary work locations will be as follows:

ESSENTIAL FUNCTIONS AND DUTIES: (May perform additional duties of similar complexity within SCHS as required or assigned)

Clinical Expectations:

  • Practices and role model excellence in the Immediate Care domains of diagnosis, aligning treatment goals to patient values, definitive treatment, supportive care treatment, follow-up care, promote good health and prevent illness.
  • Practices medicine within the scope of license, training and credentialing. Cooperate with other SCHS providers and caregivers to insure optimal patient care.
  • Assesses, plans, implements and evaluates individual patient care while demonstrating expert knowledge of evidence-based, Immediate Care.
  • Documents all patient care in the proper form and in a timely manner in compliance with hospital policies and regulatory agencies.
  • Delivers first-rate customer service and/or patient care in a manner that promotes goodwill, and is timely, efficient and accurate, and courteous.
  • Calculates dosages and orders medications accurately in accordance with SCHS policies and procedures for best practices for of medication administration.
  • Diagnoses and manages minor illness and injury including children; stabilizes and provides appropriate transfer of patients requiring higher level of care; performs emergency patient codes per ACLS and PALS guidelines.
  • Repairs lacerations; incisions and drainage of abscesses and treatment of skin infections; diagnosis and initial management of fractures and dislocations; foreign body removal; ingrown toenail removal; diagnosis and treatment of eye infections and foreign bodies (including use of slit lamp).
  • Performs initial interpretation of x-ray; EKG interpretation; review of returning lab and x-ray reports and assigning appropriate dispositions and follow-up.
  • Willingness to provide short term medication refills (non-narcotic or scheduled) for patients in provider transition; review of appropriate prescription monitoring data bases and conforming to our policy of limited narcotic prescribing.
  • Provides a therapeutic presence in service to others by purposefully responding to the needs of patients/customers in a caring way. This includes introducing oneself and stating your purpose, asking the patient their preferred name, spending time with the patient to determine their care goals, active listening, and using skillful communications (appropriate touch, eye contact, etc.).
  • May perform additional duties of similar complexity within the organization, as required or assigned.

Administrative Expectations:

  • Documents all patient care within the Electronic Medical Record (EMR) and completes all necessary tasks in compliance with health system policies and regulatory agencies to ensure timely and accurate billing.
  • Cognizant of SCHS and SCMG organizational structures.
  • Participates in SCMG provider meetings and assist in policy formation for SCMG and provider practice.
  • Maintains current clinical knowledge and skills through regular CME activity. Must attest to having adequate (CME) to meet maintenance of board certification and compliance with MSO rules and continuing privileges without restriction.
  • Maintains current and unrestricted licensure and appropriate certifications to maintain privileges at a St. Charles Health System hospital sufficient to permit performance of job duties.
  • Participate in an annual provider 360 performance review.

  • Acts as a leader and supports departmental goals and the vision, mission and values of the St. Charles Health System in all respects.
  • Supports Value Improvement Practice (VIP- Lean) principles of continuous improvement with energy and enthusiasm, functioning as a champion of change; Contribute as necessary to assist in policy formation for the organization.
  • Maintains compliance of hospital's Medical Staff Bylaws, Rules and Regulations, as required by SCHS Board of Directors.

EDUCATION

Required:

  • Graduation from an accredited medical school.
  • Successful completion of an accredited residency program in emergency medicine or family medicine
  • Successful completion of additional educational programs necessary to enable Physician to acquire and maintain privileges to perform the range of services contemplated. This includes board eligibility/certification in the specialty being practiced.
  • Complete annual fire/safety, etc. education in-services as required.
  • Attend mandatory educational offerings as described in SCHS policies/procedures and/or as required by other regulatory bodies.
  • Complete annual CME as required to maintain license and appropriate Board certification.
  • Complete by assigned due date Computer Based Learning modules.

LICENSURE/CERTIFICATION/REGISTRATION

Required:

  • Current license as MD / DO.
  • Board certified or eligible in Physician's chosen specialty. Those applicants who are not board certified at the time of application but who have completed their residency or fellowship training are required to become board certified within seven years of residency or fellowship training.
  • Unrestricted Federal DEA number.
  • Unrestricted ability to participate in Medicare / Medicaid, or other federal or state governmental health care programs.
  • Valid driver's license and the ability to meet SCHS driving requirements.
  • Current BLS certification required.
  • Current ACLS and PALS certification required.
  • Additional certifications may be required based on privileges granted to provider by Medical Staff Services at SCHS Inpatient locations.

Preferred:

  • Oregon license as MD / DO.
  • Federal DEA number registered in Oregon.

EXPERIENCE

Required:

  • Experience using EMR (Electronic Medical Records) software.
  • Advanced written and verbal communication skills to effectively interact with a diverse population and professionally represent St. Charles Health System.
  • Experience working in a team environment and proven ability to collaborate, manage facilitation and consensus building among health care professionals and agencies to achieve expected results.

Preferred:

  • 2 years of medical practice in emergency medicine, urgent care, or primary care with substantial acute care component
  • Epic EMR software experience.

PERSONAL PROTECTIVE EQUIPMENT

  • Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.

GENERAL
