This position provides perinatal and child health nursing services as a nurse home visitor. Programs include CaCoon (case management for children with special health care needs), Babies First (pre-, post-natal and children 0-5), Family Connects (universally-offered post-natal) and other nurse home visiting programs as appropriate. The position requires providing home visiting services for eligible women and their families in Deschutes County. Duties include but are not limited to performing nursing assessments to evaluate the health status of pregnant women and children; identifying and referring families to medical and community services; providing education for a healthy pregnancy and child development; documenting visit encounters to include nursing assessment, nursing diagnosis, planning, implementation, and evaluation of care; providing case management and consultation with WIC and perinatal care staff on client needs; coordinating care with primary care providers and other key partners by promoting prevention programs in their practices; and serving as a link between primary care and public health. This position may require travel throughout Deschutes County to include Bend, Redmond, Sisters and La Pine.This job posting is not meant to be an all-inclusive list of duties and responsibilities, but constitutes a general definition of the position's scope and function. Compensation $5,305.29 to $7,109.58 per month for a 172.67 hour work month. Excellent Deschutes County benefit package when eligible. This union-represented position is available immediately. Minimum Qualifications Bachelor's degree in nursing and two years of experience in public health nursing or related nursing experience in public or community health OR an Associate's degree in nursing and four years of experience in public health nursing or related nursing experience in public or community health. Must possess a Registered Nurse license in the State of Oregon or have the ability to obtain date of hire. Preference will be given for: + A Bachelor's degree in nursing with two years of public health or maternal child health nursing experience. + Bilingual in English and Spanish. Must have a demonstrated ability to: + Communicate effectively both verbally and in writing. + Establish and maintain effective working relationships with community agencies, the general public, and other employees at all times. + Maintain confidentiality. + Work independently as well as in a team setting. + Show understanding of cultural competency, time management, and problem solving skills. Necessary Special Requirements Possession of or ability to obtain a valid Oregon driver's license within 30 days of hire date. The employment offer will be contingent upon presentation of an acceptable and verifiable driver's license, pre-employment screening for criminal history, driving history, and controlled substances (NOTE: Positive test results for marijuana use may result in rescission of a contingent offer of employment). This screening must be completed with satisfactory findings in order for a formal offer of employment to be extended. Must have dependable transportation and be willing and able to drive throughout Deschutes County in sometimes adverse weather conditions. Must be willing to work a flexible schedule when necessary to meet the needs of the department. Possession of or ability to obtain an Oregon State Registered Nurse License by time of hire. Sufficient ambulatory ability to deliver services throughout Deschutes County, including the materials needed for presentations, and to provide face-to-face services in a variety of settings in the community, including but not limited to hospitals, schools, jails, clinics, client homes, and community and residential settings within required response times. Special physical requirements: + Lift and carry equipment up to, but not exceeding 30 pounds. + Climb stairs, stand for extended periods, walk distances of 100 feet, and sit down on the floor and get back up without assistance.
