Opportunity The Chief of Primary Care will provide clinical
leadership and mentorship for the Clinic Medical Director team
within the full spectrum of quality, safety and value initiatives
of Mosaic Medical. This position will work in partnership with the
CCIO, COO, and other department and organizational leaders to align
efforts, improve processes, and demonstrate quality care and value
to our patients and community. Responsibilities
- Partner with organizational leaders, including but not limited
to Value Improvement, Operations, Behavioral Health, Nursing, etc.
in developing strategy to advance value related aspects of clinical
care, including but not limited to, population health management,
clinical quality outcomes, patient experience, patient access,
provider efficiencies, team care, and cost of care.
- Review and synthesize current and emerging evidence-based
literature relevant to Mosaic clinical programs and quality
initiatives serving as a clinical resource for Mosaic clinics and
- In partnership with the CCIO and COO, strategize and innovate
in how to improve the approaches to how we interact with and engage
in care with our population with attention creating the most value
for both the patients as well as the team.
- Collaborate with CCIO in establishing clinical focus areas
(e.g. standards of care, quality improvement indicators, and
patient care protocols) and supports Clinic Medical Directors
(CMDs) in implementing and monitoring these focus areas.
- Actively participate in the performance improvement program and
monitors quality and appropriateness of care with CMDs including:
- Patient satisfaction results
- Patient/clinical incident reporting trends
- Medical record reviews (chart audits)
- Peer Review Process in partnership with Clinicians
- Serve as clinician lead, and works in partnership with the
Value Improvement Director in meeting Quality Assurance Plan
objectives and committee.
- Partner with the CCIO and COO in addressing the root causes of
burnout in providers and other team members.
- Serve as an active member of the senior leadership team and
attends other administrative meetings as required.
- Provide direct patient care including preventive care, primary
diagnosis and treatment in a primary care clinic setting.
- Continue to learn about, grow in, and practice daily the
leadership philosophy and the Mosaic Leadership Manifesto.
- Use current evidence-based guidelines in creating diagnostic
and treatment plans; utilizing tools such as "Up to Date" or
Skills & Knowledge Understands and has experience with fiscal
accountability and budgeting. Knowledge of quality infrastructure,
including how to develop and implement decision tools, clinical
protocols and guidelines, care management programs, and outcome
measurement assessments. Knowledge of information technology,
including statistical analysis, clinical epidemiology, and medical
informatics; understanding best practices and how to successfully
introduce information technology into clinical practice. Excellent
interpersonal and communication skills, with the ability to build
consensus with the ability to engage clinicians and staff.
Knowledge of medical coding, both ICD-10 and CPT. Qualifications
Education Preferred Doctorate/Phd or better in Doctor of Medicine.
Masters or better in Nursing. Experience Preferred 10 years: A
minimum of five years of experience in progressive clinical
leadership roles in a primary care medical group or community
health center environment required. Two years of Executive
leadership experience highly preferred. Ten years of clinical
experience in ambulatory care required, preferably in a community
health center with integrated care.
