This position is located in a state of the art outpatient primary care clinic at the hospital campus with colocation of internal medicine and family care practitioners. St. Charles Medical Group is a comprehensive network of more than 240 providers that covers a complete range of primary care as well as more than 20 sub-specialties.POSITION OVERVIEW: St. Charles Medical Group, Bend OR is seeking an outpatient-only internal medicine APP to assist in managing 3 existing internal medicine physician patient panels and providing primary care as part of a comprehensive CPC plus medical home.Work schedule: Provider will work minimum of 40 hours per week. This will generally be four 10-hour days scheduled as a blend of scheduled clinic visits, alternative visits, visiting skilled nursing facilities, telephone f/up, and overall panel management.Call expectation: The provider is expected to participate in the clinic call rotation which is shared equally by all providers in the Bend, Sisters, and La Pine primary care clinics. 1:15 Telephone Call.Work location: Physician's primary work location Bend Internal Medicine at 2600 NE Neff Rd.ESSENTIAL FUNCTIONS AND DUTIES: (May perform additional duties of similar complexity within SCHS as required or assigned)Clinical Expectations:* Practice and role model excellence in the Internal Medicine domains of diagnosis, aligning treatment goals to patient values, definitive treatment, supportive care treatment, follow-up care, promote good health and prevent illness.* Practice medicine within the scope of license, training and credentialing. Cooperate with other SCHS provider and caregivers to insure optimal patient care.* Assess, plan, implement and evaluate individual patient care while demonstrating expert knowledge of evidence-based, primary care.* Collaborate with other members of the Internal Medicine care interdisciplinary team and referring providers to ensure a world-class patient experience.* Calculates dosages and administers medications accurately in accordance with SCHS policies and procedures for medication administration.* Document all patient care in the proper form and in a timely manner in compliance with hospital policies and regulatory agencies.* Participates equally in identified clinic's call duties and responsibilities.* Performs surgical procedures commensurate with surgical competency.* This position will not be establishing a patient panel but rather assisting in co-management of 3 existing Internal medicine physician patient panels.* Provider on occasion will travel to care facilities with in central Oregon to provide alternative care visits.* Provider maybe expected to manage E-visits depending upon patient volumes.Administrative Expectations:* Document all patient care within the Electronic Medical Record (EMR) and complete all necessary tasks in compliance with health system policies and regulatory agencies to ensure timely and accurate billing.* Be familiar with the SCHS organization structure.* Participate in SCMG physicians meetings and assist in policy formation for SCMG and Physician's practice.* Keep medical knowledge and skills current through regular CME activity. Must attest to having adequate (CME) to meet maintenance of board certification and compliance with MSO rules and continuing privileges without restriction.* Keep all licenses and appropriate certifications current and unrestricted.* Maintain active privileges at a St. Charles Health System hospital sufficient to permit performance of job duties.Behavioral Expectations:* Participates in creating a healing environment that supports all aspects of the care environment and the wholeness of each individual, patient and caregiver.* Provides and maintains a safe environment for caregivers, patients and guests. Conducts all activities with the highest standards of professionalism and confidentiality.* Delivers first-rate customer service and/or patient care in a manner that promotes goodwill, and is timely, efficient and accurate, and courteous.* Educates, counsels, and assists patients and families regarding current health problems, health maintenance activities and preventive behaviors.* Provide a therapeutic presence in service to others by purposefully responding to the needs of patients/customers in a caring way.* This includes introducing oneself and stating your purpose, asking the patient their preferred name, spending time with the patient to determine their care goals, active listening, and using skillful communications (appropriate touch, eye contact, etc.).* Complies with all applicable laws, regulations, policies and procedures, supporting the health system's corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violations of applicable rules, and cooperating fully with all health system's investigations and proceedings.EDUCATIONRequired:* Successful completion of an approved Physician Assistant and/or Nurse Practitioner course of study and certification as appropriate to the incumbent's license.* Attend mandatory educational offerings as described in SCHS policies/procedures and/or as required by other regulatory bodies.* Complete annual CME as required to maintain license and appropriate Board certification.* Complete in a timely manner assigned Computer Based Learning modules.LICENSURE/CERTIFICATION/REGISTRATIONRequired:* Unrestricted Physician Assistant and/or Nurse Practitioner licensure.* Physician Assistant requires NCCPA certification. Nurse Practitioner requires AANC, NCC or AANP, AMCB certification. If Physician Assistant, ability to establish and maintain a Practice Agreement with a Supervising Physician through OMB.* Unrestricted Federal DEA number.* Unrestricted ability to participate in Medicare / Medicaid, or other federal or state governmental health care programs.* Valid driver's license and the ability to meet SCHS driving requirements.* Current BLS certification required.* ACLS certification is required after 6 months of employment if assisting in infusion room oversite.* Additional certifications may be required based on privileges granted to provider by Medical Staff Services at SCHS Inpatient locations.Preferred:* Oregon licensure.* Federal DEA number registered in Oregon.EXPERIENCERequired:* 0-2 years' experience as an Advance Practice Provider.* Experience using EMR (Electronic Medical Records) software.* Advanced written and verbal communication skills to effectively interact with a diverse population and professionally represent St. Charles Health System.* Experience working in a team environment and proven ability to collaborate, manage facilitation and consensus building among health care professionals and agencies to achieve expected results.Preferred:* 2 plus years' experience as an Advance Practice Provider in Internal Medicine or Primary care.* Epic EMR software experience.PERSONAL PROTECTIVE EQUIPMENTMust be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.GENERAL* Must have excellent communication skills and ability to interact with a diverse population and professionally represent SCHS.* Strong team working and collaborative skills.* Ability to multi-task and work independently.* Attention to detail.* Excellent organizational skills, written and oral communication and customer service skills, particularly in dealing with stressful personal interactions.* Intermediate to advanced proficiency with Electronic Medical Record systems.* Intermediate in Microsoft applications (Word, Excel).PHYSICAL REQUIREMENTS:* Continually (75% or more): Use of clear and audible speaking voice and the ability to hear normal speech level.* Frequently (50%): Sitting, keyboard operation.* Occasionally (25%): Standing, walking, bending, stooping/kneeling/crouching, climbing stairs, reaching overhead, lifting/carrying/pushing or pulling 1-10 pounds, grasping/squeezing, operation of a motor vehicle, ability to hear whispered speech level.* Never (0%): Climbing ladder/step-stool, lifting/carrying/pushing or pulling 11-50 pounds.* Exposure to Elemental Factors* Occasionally (25%): Chemical solution for Laboratory Caregivers.* Rarely (10%): Wet/slippery area.* Never (0%): Heat, cold, noise, dust, vibration, chemical solution, uneven surface.* Blood-Borne Pathogen (BBP) Exposure Category* Risk for Exposure to BBPSchedule Weekly Hours:40Caregiver Type:RegularShift:Is Exempt Position?YesJob Family:NURSE PRACTIONER SCMG, PHYSICIAN ASSISTANT SCMGCompensation (min.):0/ HourCompensation (mid):0/ HourCompensation (max):0/ HourScheduled Days of the Week:Shift Start & End Time:
