As a Family Practice Nurse Practitioner, you will play a crucial role in delivering comprehensive assessments, managing chronic conditions, and supporting patient health directly within the community. Required Skills: Current and active Oregon Medical License Certification as a Nurse Practitioner with family practice specialization Proven experience in conducting health assessments and managing chronic conditions Excellent communication and interpersonal skills Ability to work independently and collaboratively Proficient in documentation and electronic health records (EHR) systems Valid driver's license and reliable transportation Nice to Have Skills: Experience with telehealth platforms and remote patient monitoring Bilingual abilities to serve diverse populations Prior experience in home health or community-based care Familiarity with Medicare and Medicaid guidelines Preferred Education and Experience: Master's Degree in Nursing (MSN) or higher from an accredited institution Minimum of 2 years of clinical practice as a Nurse Practitioner Certification in family medicine or primary care Other Requirements: Enrolled and credentialed in Medicard Ability to travel to patient homes within designated counties Willingness to work daytime shifts, Monday through Friday (08:00 – 17:00) with 1-hour lunch Commitment to completing ongoing education during work hours Reliable transportation for travel reimbursement at IRS standard rates Availability to start on 03/09/2026 This is an excellent opportunity to join a compassionate team committed to enhancing patient well-being through innovative healthcare delivery.
