the Nurse Practitioner, you will have the opportunity to
collaborate with the Clinical Team to contribute to that mission.
These passionate team members will depend on you to provide
comprehensive care to hospice patients, and to be a reliable go-to
problem solver.On an average day, you willProvide oversight of
clinical and Nurse Practitioner services in coordination with the
Medical DirectorAssist with on-call needs when the Medical Director
is unavailableProvide oversight on initial and ongoing assessment
of the impact of the terminal diagnosis on daily living, and the
patient's physical, functional, psycho-social, and environmental
needsPartner with an interdisciplinary team to implement and manage
adequate, individualized plans of care, determining scope,
priority, and frequency of services based on acuity and
patient/family needsAnd perform additional, related duties as

1 year of Nurse Practitioner work experience, preferably in
hospiceHave an active Advanced Practice Registered Nurse (APRN),
