Services include skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work, aide, and phlebotomy.POSITION OVERVIEW: The Nurse Manager is responsible for providing oversight of all patient care services and Home Health Agency personnel. Oversight includes making patient and personnel assignments; coordinating patient care; coordinating referrals; and assuring the development, implementation, and updates of the individualized plan of care. Participates in planning, budgeting, staffing, performance, and operation of Home Health in conjunction with the Administrator as well as performance and annual review of direct reports. Consults with staff, physicians and administrative members on clinical problems and interpretation of hospital policies to ensure patient needs are met. This position directly manages caregivers in Home Health.ESSENTIAL FUNCTIONS AND DUTIES:Provides leadership and direction to the home health department and effectively manages day-to-day clinical operations.Performs planning and coordination of workflow. Brings a global perspective to the team.Manages clinical resources for the agency.Sets expectation of performance of clinical staff assigned to the department. Provides for the development of supervisors/caregivers through effective advising, coaching, facilitating, and mentoring.Ensures consistent and technical excellence and compliance, superior service, and the integration of the healing health care principles.Accountable for the long term strategic performance of the department including the identification of business development opportunities and the successful execution of strategic initiatives.Develops and implements annual plan including goals for department with director, managers and supervisors.Manages the department according to the budget and operating plan ensuring financial accountability and productivity in all operational aspects.Responsible for recruitment, hiring, orientation, discipline, and evaluation of all clinical caregivers. Maintain adequate staffing levels and coverage during staffing absences as necessary. Comply with all St. Charles Healthcare Human Resource policies and procedures.Monitors and ensure all direct reports are current with compliance and safety requirements. Implements and manages all organizational safety directives and goals.Provides and oversees team's delivery of customer service in a manner that promotes goodwill, is timely, efficient and accurate.Serves as a real-time resource and assists with clinical expertise for staff with problem-solving on patient care, processes, and family issues. Serves as representative for all customers.Communicates (both written and orally) effectively and maintains professional relationships with caregivers, medical staff, patients, guests and other hospital departments while providing 2 way communication of the SCHS mission and strategic plan.Oversees department's practices to meet regulatory agency requirements. Oversees staff documentation for patient care to ensure proficiency in compliance with hospital policies and regulatory agencies.Supports the vision, mission and values of the organization in all respects.Supports Value Improvement Practice (VIP- Lean) principles of continuous improvement with energy and enthusiasm, functioning as a champion of change.Provides and maintains a safe environment for caregivers, patients and guests.Conducts all activities with the highest standards of professionalism and confidentiality. BSN or Bachelor's degree in related field.Preferred: Master's degree in NursingLICENSURE/CERTIFICATION/REGISTRATIONRequired: Current Oregon RN licensure with the Oregon State Board of Nursing.Valid Oregon driver's license and ability to meet SCHS driving requirements. Ability to travel to business functions/trainings/meetings and all SCHS worksites.Preferred: N/AEXPERIENCERequired: Five years hospital, skilled nursing facility, and outpatient or rehabilitation experience required with two years' experience in Home Health. 1 year of leadership experience.Preferred: 2 years of nurse management experience.PERSONAL PROTECTIVE EQUIPMENTMust be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.PHYSICAL REQUIREMENTS:Continually (75% or more): Use of clear and audible speaking voice and the ability to hear normal speech level.Frequently (50%): Sitting, standing, walking, lifting 1-10 pounds, keyboard operation.Occasionally (25%): Bending, climbing stairs, reaching overhead, carrying/pushing or pulling 1-10 pounds, grasping/squeezing.Rarely (10%): Stooping/kneeling/crouching, lifting, carrying, pushing or pulling 11-15 pounds, operation of a motor vehicle.Never (0%): Climbing ladder/step-stool, lifting/carrying/pushing or pulling 25-50 pounds, ability to hear whispered speech level.Exposure to Elemental FactorsNever (0%): Heat, cold, wet/slippery area, noise, dust, vibration, chemical solution, uneven surface.Blood-Borne Pathogen (BBP) Exposure CategoryNo Risk for Exposure to BBP.Schedule Weekly Hours:40Caregiver Type:RegularShift:First Shift (United States of America)Is Exempt Position?YesJob Family:MANAGERScheduled Days of the Week:As Scheduled (may include weekends and holidays)Shift Start & End Time:
