CHARLES HEALTH SYSTEM* JOB DESCRIPTION TITLE: Pathway to Excellence Program Coordinator REPORTS TO POSITION: Chief Nursing Officer DEPARTMENT: Patient Care Support DATE: November 5, 2021 OUR VISION: Creating America's healthiest community, together OUR MISSION: In the spirit of love and compassion, better health, better care, better value OUR VALUES: Accountability, Caring and Teamwork DEPARTMENT SUMMARY: The Patient Care Support Department provides 24/7 house wide support to all departments and care to patients during their stay at St. Charles Madras through hospitalization or discharge. POSITION OVERVIEW: Under the general direction of the Chief Nursing Officer, the Pathway to Excellence Program Coordinator (PEC) is responsible for leading the St. Charles Madras Hospital's (SCM) initiative to seek the American Nurse Credentialing Center's (ANCC) status as a Pathway Hospital. The PEC is responsible for the development, implementation, communication, and project management of activities related to this initiative. Upon achievement of Pathway status, the PEC is responsible for ongoing maintenance of status. ESSENTIAL FUNCTIONS AND DUTIES: Integration of Pathway Standards Facilitates nursing leadership understanding of and integration of Pathway to Excellence standards. Promotes department strategic planning that addressed implementation of standards. Facilitates periodic self-assessment or gap analysis and improvement planning to strengthen integration of standards. Stays apprised of the ANCC's revisions and clarifications regarding standards. Communicates revisions and clarifications to nursing leadership and other appropriate personnel. Pathway Office Liaison Maintains open communication with ANCC's Pathway to Excellence Office personnel. Confirms and communicates changes and revisions in ANCC Pathway policies and procedures. Maintains accurate and current files of all ANCC correspondence. Once Pathway designated, coordinates all required activities with ANCC and responds to queries from other organizations seeking Pathway designation. Education and Community Relations Develops and coordinates Pathway-related educational materials and presentations for all SCM caregivers. Mentors nursing caregivers to support their role as educators regarding the Pathway process. Develops and coordinates the Pathway committee. Provides ongoing education about the Pathway standards and principles of a professional practice environment to support the goals of SCHS. Serves as an educational resource to the nursing division regarding the Pathway process. Works closely with Communications and Marketing to produce Pathway-related publicity or advertisement materials. Coordinates celebrations and award ceremonies and displays in collaboration with Communications and Marketing. Application and Reapplication process, Data management Chairs and coordinates meetings and activities of the Pathway committee. Ensures timely compliance with procedures for submission of all materials required for application and reapplication. Coordinates efforts to identify the appropriate evidence for each standard or indicator. Coordinates collection, categorization and filing of all Pathway evidence. Coordinates preparation, editing, duplication, and publication of all Pathway evidence as it pertains to the standards. Prepares periodic reports and updates on progress toward designation or re-designation. Once Pathway designated, prepares and files all required ongoing documentation. Intra/Interdepartmental Collaboration Collaborates with department managers to educate caregivers. Provides periodic updates to SCM managers on the Pathway process. Facilitates integration of Pathway standards as needed across all departments. Collaborates with Licensing and Accreditation department to assure integration the Pathway standards as they relate to regulatory standards. Miscellaneous Communicates (both written and orally) effectively and maintains professional relationships with caregivers, medical staff, patients, guests, and other hospital departments. Fosters teamwork with all shifts and other units/departments. Supports the vision, mission, and values of the organization in all respects. Supports Value Improvement Practice (VIP- Lean) principles of continuous improvement with energy and enthusiasm, functioning as a champion of change. Provides and maintains a safe environment for caregivers, patients, and guests. Participates in creating a healing environment that supports all aspects of the care environment and the wholeness of each individual, patient and caregiver. Conducts all activities with the highest standards of professionalism and confidentiality. Complies with all applicable laws, regulations, policies, and procedures, supporting the organization's corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violation of applicable rules, and cooperating fully with all organizational investigations and proceedings. Delivers customer service and/or patient care in a manner that promotes goodwill, is timely, efficient, and accurate. Facilitates the growth and development of a culturally competent team in the provision of appropriate care to patients and family members who belong to diverse cultural backgrounds. May perform additional duties of similar complexity within the organization, as required or assigned. EDUCATION Required: Bachelor's Science Nursing (BSN) Preferred: Master's Science Nursing (MSN) LICENSURE/CERTIFICATION/REGISTRATION Required: Current Oregon RN license. AHA Basic Life Support for Healthcare Provider certification. Preferred: N/A EXPERIENCE Required: Three years clinical hospital experience. ANCC Pathway to Excellence training. Preferred: Experience working in a Pathway status organization. PERSONAL PROTECTIVE EQUIPMENT Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely. ADDITIONAL POSITION INFORMATION: General: Must have excellent communication skills and ability to interact with a diverse population and professionally represent St. Charles Health System Excellent organizational and multitasking skills. Strong teamwork and collaborative skills. Strong analytical, problem solving and decision making skills. Basic to intermediate ability and experience with computer applications, specifically electronic medical records system and MS Office. PHYSICAL REQUIREMENTS: Continually (75% or more): Use of clear and audible speaking voice and the ability to hear normal speech level. Frequently (50%): Sitting, standing, walking, lifting 1-10 pounds, keyboard operation. Occasionally (25%): Bending, climbing stairs, reaching overhead, carrying/pushing or pulling 1-10 pounds, grasping/squeezing. Rarely (10%): Stooping/kneeling/crouching, lifting, carrying, pushing or pulling 11-15 pounds, operation of a motor vehicle. Never (0%): Climbing ladder/step-stool, lifting/carrying/pushing or pulling 25-50 pounds, ability to hear whispered speech level. Exposure to Elemental Factors Never (0%): Heat, cold, wet/slippery area, noise, dust, vibration, chemical solution, uneven surface.
