This and other physician assistant jobs brought to you by DocCafe.com **Internal Medicine Nurse Practitioner Opening in Oregon** * Located in ***Bend, OR*** * Full-time, permanent position Opportunity: This position serves patients with complex medical, psychological and social issues in an outpatient setting. This patient population consists of a high amount of mental health and substance abuse as these patients are assigned to our Bridges Health clinic through a referral program after meeting specific criteria. This is a 30 hour per week position. Responsibilities: Provides direct patient care within the scope of primary care medicine in an outpatient, and in certain circumstances inpatient settings. Examines, diagnoses, and treats acute and chronic medical conditions. Provides care which reflects health care cost consciousness and addresses social determinants of health. Effectively utilizes team based care model, leveraging expertise of all members of multi-disciplinary team. Maintains accurate electronic health records of patients and completes documentation in timely manner.
