SCHS CC bridges the needs of the patients as they traverse across the continuum of care and throughout the life cycle providing care and supportive services to our central and eastern Oregon communities. CC is committed to providing safe, timely, equitable and patient centered care and strives for excellence in care, technology, and services. These services provide continuity of care and encourage a focus on providing care for common conditions and outpatient care, while developing strong relationships, service line collaboration and referrals of higher acuity conditions.--Patient care is provided to patients and families as part of an integrated system of settings, services, and care levels that make up a continuum of care with the goal of helping our patients live as independently as possible with a higher quality of life.The AIM Program provides palliative Care services to St. Charles Health System. Our Advanced Illness Management (AIM) Program is designed to help patients and their families maximize physical, emotional and spiritual quality of life and ensure their wishes for their healthcare are honored utilizing interdisciplinary approach to care.Hospice care focuses on the care of the terminally ill to manage pain and symptoms while also attending to their emotional and spiritual needs. Supporting patients in life transitions.--Home Health: Home health care is supportive care provided in the home. Care is provided by licensed healthcare professionals who provide medical treatment needs or by professional caregivers who provide daily assistance to ensure the activities of daily living are met. Specialized home health care services help patients and families find solutions for maintaining a healthy lifestyle by providing support and specialized services that are delivered directly in the home environment.--ESSENTIAL FUNCTIONS AND DUTIES: Accountable for strategic planning and execution, clinical and financial outcomes, assuring the highest quality patient satisfaction and service, facilitating a healthier community.--Provides strategic thinking and planning to identify the areas in which collaboration could provide optimal benefit and innovative opportunities within the Continued Care Division, SCHS, and throughout the community and stateWorks with Medical Director, key administrators, including advisory committees, other community and SCHS departments to provide advice and support that relates to the service and financial success of the health system.Develops and manages integrated nursing and clinical services to meet regulatory requirements (CMS, Joint Commission, and OHA) and the needs of the patient.--Demonstrate advanced abilities in accounting management, including financial reporting and analysis as well as operational and capital planning. To include the development and responsibility for the department budgets as well as development of the business plan for future growth.Effectively develops/ administers all budgets of the departments and is accountable for the financial performance of the departments within the SCHS parameters.--Manages financial analysis function as required for SCHS strategic and business planning.Creates a healing environment that supports all aspects of care, respects the wholeness of the individual, and demonstrates therapeutic presence through attitudes and behaviors that enhance the care experience for patients and caregivers.----Coordinate operations with hospital campus's and system departments to provide evidenced-based clinical care with a relentless focus on the delivery high quality patient care that is safe, timely, effective, efficient, equitable, and patient-centered.Maintains a common core set of nursing leadership competencies in: communication and relationship management; knowledge of the health care environment; leadership; professionalism; business skills and principles as defined in AONE Nurse Executive Competencies 2015.Builds effective collegial relationships with applicable internal and external constituents and stakeholders ensuring optimal operating effectiveness and strategic positioning.Provides leadership and management to assigned department(s) and effectively oversees the internal operations of these departments.--Hires, directs, coaches and monitors the performance of all direct reports, to develop and maintain a high performance team that meets organizational and department goals.Implements standards that will ensure safe and effective nursing care to patients in accordance with established nursing, medical, legal, and accrediting agency standards.--Develop and implement policies and procedures that guide and support the provision of nursing and clinical services while maintaining compliance with pertinent laws and regulations.Demonstrate effective financial management with the ability to develop cost-effective solutions relating to staffing, resource allocation, budget development, and management while meeting productivity goals.Partners and builds a strong relationship with the community providers and business partners to ensure integration of care delivery, satisfaction with services and clinical competence.Assures the hiring of competent caregivers, provision of programs for proper orientation, relevant continuing education, and competency validation of caregivers.Establish and manage expectations of performance of caregivers related to compliance, safety requirements, and St. Charles Health System values of accountability, caring and teamwork.Responsible for budget development, regular monitoring, accountability and meeting all operational targets for all areas within span of controlMonitors and ensure all direct reports are current with compliance and safety requirements.--Implements and manages all organizational safety directives and goalsProvides and oversees team's delivery of customer service in a manner that promotes goodwill, is timely, efficient and accurate.Supports the vision, mission and values of the organization in all respects.Collaborates with teams to review processes and identify/implement opportunities for improvements, applying Value Improvement Practice concepts and tools.Supports Value Improvement Practice (VIP- Lean) principles of continuous improvement with energy and enthusiasm, functioning as a champion of change.Provides and maintains a safe environment for caregivers, patients and guests.Conducts all activities with the highest standards of professionalism and confidentiality.--Complies with all applicable laws, regulations, policies and procedures, supporting the organization's corporate integrity efforts by acting in an ethical and appropriate manner, reporting known or suspected violation of applicable rules, and cooperating fully with all organizational investigations and proceedings.May perform additional duties of similar complexity within the organization, as required or assigned.--------------------------------------EDUCATION--------------------------------------Required: Requires a Master's Degree in in a relevant discipline.Preferred: MSNLICENSURE/CERTIFICATION/REGISTRATIONRequired:Current Oregon RN license.Valid Oregon driver's license and ability to meet St. Charles Health System driving requirements.AHA Basic Life Support for Healthcare Provider certification required.Preferred:----------------Professional leadership certification preferred (examples:--CHPN or related leadership certificatons)EXPERIENCERequired:Requires a minimum of 8 years of current nursing leadership and management experience demonstrating progressive leadership development and competency.--Microsoft Windows suite proficiency required.Preferred:Prior experience as a senior patient care services executive is strongly preferred, but candidates who are exceptionally qualified for promotion to this position will be considered.Previous experience with an EMR Information preferred.--Previous experience which includes a balance of strategic and operational skills is an asset.Previous experience with Conditions of Participation regulations for Hospice and Home Health.
