Position Summary:Provides case management and resource
coordination through collaboration and consultation within
Bethlehem Inn's team environment, as well as coordinating with
other community partners and agencies to ensure effective treatment
and equal access for residents. Holds a case management caseload of
Contract agencies and as assigned. Works independently with general
supervision; exercises judgment within defined guidelines and
practices to determine appropriate action. - -The primary
responsibility for any staff member is the safety and security of
the facility, staff, residents and volunteers, while providing
emergency shelter and food. This requires the flexibility of all
staff members to adjust to and respond accordingly to immediate
issues of concern, whether identified by another staff member,
resident or outside representative. Teamwork and open communication
between staff is essential in the success of the Bethlehem Inn, its
residents, staff, and volunteers. This promotes a healthy and
fulfilling work environment.Essential Functions & Responsibilities:
- -(Includes the following, though other duties may be
assigned)Resource Coordinator
- Understand and assist with resource mapping that would enhance
self-sufficiency for residents and keep up to date with the process
for accessing the available resources. -
- Develops and oversees resource referral documents, partnership
agencies contact, and resource referral tracking, for accurate
referrals, resources, and supportive services.
- Partner with Case Management Coordinator, Program Support/Case
Managers and direct case management, provide practical help,
community resource referrals, and individualized support to resolve
barriers, etc., as appropriate to the needs of each
individual/household.
- Holds a case management caseload of Contracted partnerships
residents and others as assigned.
- Ensures Case Management Coordinator and case managers are up to
date with changing resources, processes for referrals, and accurate
tracking on resource referrals. -
- Ensure that Program Support/Case Managers maintain accurate,
up-to-date records and case files on the residents and report
regularly to the Program Managers. -
- Meets with Program Managers regularly to review resources
organization, partnerships, and updates. - -
- Maintains strong established professional partnerships with
outside agencies in collaboration with Program Managers. -
- Attends bi-weekly Program Meetings as scheduled; Improves staff
competence by providing educational resources
- Collaborates with Program Impact Analyst on resource referral
tracking, non-resident assistance, case management trends and data
quality. -
- Assists Program Impact Analyst with data quality concerns and
increases tracking documentation. -
- Assist and participate in Homeless Leadership Coalition tasks
as assigned by Program Managers, designated as a proxy as
needed.
- Participate in HLC committee work, Built for Zero by- list
collaboration, and reports community updates and changes to Program
Managers.
- Administers and completes resource and coordination reports as
scheduled.Team and Program Support -
- Maintains professional boundaries and utilizes de-escalation
skills with clients. -
- Holds residents accountable for their actions in accordance
with our rules and regulations and responds immediately and
thoroughly with communication, clarification, and procedure. -
- Reviews prior communication log entries at the beginning of
each shift for changes and updates; accurately documents in the
communication log regarding events taking place during shift.
- Conducts case management meeting as scheduled; logs notes in
Apricot case management file. - -
- Maintains and updates current resident file system. -
- Follow up on client exits ensuring programmatic duties are
completed.
- Conducts rounds regularly as needed, alert for
suspicious/illegal activity and to maintain site security and
safety.
- Contacts emergency/law enforcement for issues during shift and
communicates with management team regarding these issues.
- Monitors resident behavior (adults and children) upon entry and
throughout stay; alerts to conditions that are in violation of
resident policies and safety concerns and responds swiftly and
accordingly.
- Contacts supervisors in the event of extenuating circumstances
and the need for clarification.
- Fields potential intakes for safety and security and completes
intake process as needed. -
- Attend monthly staff meeting as scheduled. -
- When needed, oversees and/or maintains cleanliness in staff
common areas. - - -
- Provide coverage and assistance for team members in all tasks
as required to maintain continuity of services.
- Works as part of a team to find solutions and takes initiative
in finding other tasks that need to be completed to assist other
team members.
- Nurtures positive optimistic attitude in the staff and
communicates a sense of mission to team.
- Teams with Operations staff when doing batch breathalyzer tests
on clients. -Professionalism

resolution.Minimum Qualifications:(Qualifications applicant must
possess in order to be considered for the position; possible
exception if an applicant possesses appropriate substitute
qualifications)
- Bachelor's Degree in Social Work, Human Services, or related
fields OR Two (2) years of direct service and case management
experience to individuals experiencing homelessness, mental health,
chemical dependency, domestic violence and/or special need
populations, and experience working with and an understanding of
culturally diverse groups.
- Oregon Food Handler's Card (must be obtained within the first
day of employment).
- Ability to exercise sound, independent judgment, to maintain a
high level of confidentiality, to respond effectively to emergency
and crisis situations and resolve problems.
- Dependable and flexible.
- Ability to write clearly and concisely and to communicate
effectively in writing. -
- Ability to communicate clearly and concisely orally.
- Analytical capability, ability to work effectively with data
and identify trends, draw on history and recognize what has worked
and what has not.
- Valid Oregon driver's license required.(The requirements listed
below are representative of the knowledge, skill, and/or abilities

desired in applicants, however applicants without these

- Demonstrated ability to provide crisis intervention and to
follow procedure in crisis situations;
- Understanding the issues surrounding homelessness, mental
health, chemical dependency, and domestic violence;
- Knowledge of services available to low-income and homeless
persons in Deschutes County through Bethlehem Inn and other
agencies or the demonstrated ability to obtain such knowledge;
- Bi-lingual and / or Bi-cultural preferred. -
- Knowledge of area community resources.
- College degree in human or social service area preferred.
